Evolve offers a unique “One-Stop-Shop” support experience, with access to healthcare, social support, groups and programmes, and counselling.
Unfortunately, we cannot accommodate everybody's needs - and depending on which services you require, there may be a wait time before you can enrol.
If you’re enrolling with our services there is an onboarding process, including a 1-hour initial appointment where a relevant member of our team will get to know you, your values, strengths, and support needs a bit better. There is a waitlist for these enrolments at present, as we have a limited number of initial appointments available each week.
Our routine enrolment criteria are listed below, however please be aware that because of high demand, we may close enrolments temporarily.
Must be a NZ Citizen or Permanent Resident.
Must be aged between 10 - 23years, 11 months.
Must be living in Wellington (excludes Hutt Valley, Porirua, and Kapiti Coast), or have no fixed housing.
Must not be studying at Victoria University, Massey University, or any tertiary institution / university with a student health service.

If you’re filling out the below enquiry form on behalf of another person, please ensure that you’ve notified them and received their consent to do so.
Next steps: When you reach the top of our waitlist, our practice assistant will contact you to discuss the services that you’re hoping to access, and note down any prior health issues / concerns. They will then book you in for your initial appointment, where you’ll fill out the required enrolment paperwork. Please Note - if there are any health issues that you would prefer to discuss in-person, this can be done at your initial appointment instead.
