Prescriptions

If you have received a prescription from Evolve before, you may be able to ask for a repeat rather than coming in for another appointment. Sometimes, we will want to see you for another appointment though, to check out how things are going and make any necessary changes. To request a repeat prescription, contact reception or use ManageMyHealth.

  • 48 hours notice is required for all prescriptions. More urgent prescriptions will be considered but not guaranteed.

  • Evolve retains the right to decline to complete a request for a prescription but we will advise you of this.

  • Evolve will contact you to advise that your request has been received. If further information is required or you need an appointment we will contact you directly.

  • You will be advised when your prescription is ready. If you would like your prescription faxed to a pharmacy there may be a charge from your pharmacy for this, please check before requesting your script.
