If you have been to Evolve before and received a prescription for some medications, you may be able to phone and ask for a repeat prescription, rather than attending another appointment. Ask the doctor or nurse if this is a prescription you might be able to renew by phone. Sometimes they will want to see you for another appointment though, to check out how things are going and make any necessary changes.
Phone (04) 473 6204 and select option 2 to leave a message about a repeat prescription.
- 48 hours notice is required for all repeat prescriptions for long-term medications. More urgent prescriptions will be considered but not guaranteed. Prescriptions can be requested by text, email, phone, or at Reception. Evolve retains the right to decline to complete a request for a prescription but we will advise you of this.
- Evolve will contact you to advise that your request has been received. If further information is required or you need an appointment we will contact you directly. You will be advised when your prescription is ready to pick up.
- If you would like your prescription faxed to a pharmacy there may be a charge from your pharmacy for this, please check before requesting your script.
- Any prescription at Reception for longer than 2 weeks will be destroyed.
